Scholarship Application Checklist
In addition to completing the online application, you must provide the following materials to the Foundation or your application will be considered incomplete and therefore ineligible. Do not submit a resume. This checklist is also available on the online application form.
· Proof of U.S. Citizenship (birth certificate) or legal permanent residence status.
· Official high school or college transcript (if currently attending) that includes your fall grades
· ACT and/or SAT scores
· Current year IRS tax Form 1040 for household (if current year is not available, previous year will be accepted)
· Two (2) letters of recommendation
· Scholarship specific attachments (see below)
Some scholarships require specific attachments that must be included with your application in order to be considered for these scholarships. Within the online application, you will have the opportunity to upload these attachments to your application if you have them in an electronic format. If not, you will be required to mail/fax them to the Foundation by the deadline or your application will not be considered; attachments postmarked by the application deadline will be accepted.
Achievement through Commitment to Education (ACE) – copy of your Student Aid Report and Accuplacer Test Scores
Cameron Shawn Buchanan Memorial Scholarship– letter from Caprock High School golf coach as verification of your participation on the golf team for a minimum of two (2) years
CareNet Pregnancy Center Scholarship in Memory of Jackie Hall – letter from the CareNet director or staff to verify you are a current and/or former client of the Center
Jake Jones Memorial Scholarship for the Learning Disabled – letter of verification from special education teacher/coordinator stating your disability
Love Your Neighbor Scholarship– letter from pastor or youth minister
Ryan James Maxwell Memorial Scholarship – letter from sponsor as proof of membership in Borger Fellowship of Christian Athletes
John McGuire Memorial Scholarship – if you have been nominated to apply for this scholarship, you must also submit two letters of recommendation from coaches and one letter of recommendation from a teacher or administrator
Clifton McNeely Endowment Scholarship – two letters of recommendation from coaches and one letter of recommendation from a teacher or administrator
Dr. Charles & Phyllis Seward Scholarship– copy of Bureau of Indian Affairs registration number or Tribal Affiliation and roll number
Charles Sheldon Memorial Scholarship – letter from vocational agriculture teacher or county agent indicating the number of years you have participated in 4-H and/or FFA activities
Fred Woodward Endowed Scholarship – letter from parent’s employer verifying membership in the Texas Seed Trade Association
